Service Description

At Melton Community Health Centre this is primarily a HACC (Home and Community Care) funded service for people receiving pension benefits, who are frail, elderly, disabled, or a carer.

Access to other community residents is prioritised to people who have diabetes or a chronic illness, and who may have a health care card or pension benefits.

A full diabetes assessment service is offered to clients and a comprehensive podiatric service is available, including orthotic therapy and nail surgery. Group education sessions are provided for people with diabetes in conjunction with the dietitian.

Eligibility

Melton CHC - All residents of Melton and Moorabool Shires who meet the strict HACC criteria. Community health priority is given to people with diabetes or severe foot pathologies.

Melton Health – Acute care of all members of the community. Melton Health provides podiatry care to those members of the community who are not eligible for care under community health funding. The podiatrist is involved in the Endocrinology and Diabetes Program, the Orthopaedics Program and the General Medicine Program.

9.00am – 5.00pm by appointment only.

Referral process

Self-referral or from General Practitioner, Medical Specialist or other health professional.

Cost to client

$10.00 for concession card holders. $15.00 for non-concession clients. Exemptions can be made following discussion with the podiatrist.

Melton Health patient consultations are bulk-billed. Orthotics however at paid for by the patient.
