Service Description

At Bacchus Marsh, Physiotherapy is offered in a hospital setting to the Pre Admission Clinic, acute inpatients, maternity services, Hospital in the Home program, residential care and outpatients, with some selected home visits.

In the acute sector the physiotherapist is responsible for assessing and treating a wide range of medical and surgical patients, including accepting referrals from those with respiratory, orthopaedic and general mobility problems. Outpatients seen have a variety of conditions and include those recently discharged from a public hospital as well as from General Practitioners.

Residents in Grant Lodge are assessed when first admitted and then on an annual basis. Programs are set by the physiotherapist, and carried out by the Allied Health Assistant, with the aim of maintaining the resident’s optimal physical function as long as possible.

At the Melton Community Health Centre, a variety of conditions are treated in the primary care setting. Hydrotherapy is conducted at Melton Waves.

At Melton Health Physiotherapists provide a comprehensive initial rehabilitation service for acute conditions aiming to restore each patient’s functional independence within the limitations of their condition or injury. Physiotherapists are involved in the Orthopaedic, Chronic Disease Management, Day Rehabilitation and General Medicine programs. They work closely with the Physiotherapists at Community Health to ensure care is provided to the patient at the most appropriate site for each patient’s needs.

Eligibility

Bacchus Marsh: any person who has been admitted to the hospital or is suitable for outpatient treatment.

Melton Community Health: HACC & Community Health

Melton Health: Any person who has been referred from a GP, hospital, or other Healthcare Provider.

Bacchus Marsh: Mondays-Fridays 8.30am – 4.30pm

Melton CHC: Mondays-Fridays 9.00am – 4.30pm

Melton Health: Monday-Friday 9.00am – 4.30pm

Referral process

By Medical Specialist, General Practitioner, other Health Professionals or hospital.

Melton Health accepts self-referrals for acute injuries.

Melton CHC: $10.00 per visit

Melton Health – Bulk Billed
