Service Description

Home and Community Care (HACC)(Melton & Bacchus Marsh)

The occupational therapist (OT) will visit individuals in their home and complete a home and functional assessment with them to identify any activities which they may be experiencing difficulty with. The OT can recommend and prescribe aids, equipment or home modifications which may assist the individual to continue to live safely and independently in their home. Recommendations include scooters, wheelchairs, personal alarms, rails and ramps. The OT can also provide individuals with information and strategies to reduce falls and teach them simplified techniques to decrease tiredness. Aids and Equipment Program (VA&EP) funding applications for recommended equipment or home modifications can also be completed if required.

Paediatric (Melton)

Development assessment with particular emphasis on fine motor and visual perceptual issues. School readiness.

Eligibility

HACC OT -  all individuals who are aged or have a disability.    Paediatric-children from 0-8 years without diagnosed disability

Access hours

Mon, Tues, Thurs 9.00am – 4.00pm (Bacchus Marsh) HACC OT

Monday – Friday 9.00am – 5.00pm (Melton) HACC and Paediatric

Referral process

HACC OT - Individuals can refer themselves to this service by calling Melton Community Health Centre (8746 1100) and asking for an OT assessment. Referrals are also accepted from General Practitioners and other health professionals via fax. Please be aware there is a waiting list for this service.
