| Service Description | The HMHB outreach worker provides support for pregnant women to access antenatal and postnatal services, and other social support services, eg housing, welfare, transport, peer support groups. Also provides information to support healthy behaviours in pregnancy and beyond, eg breastfeeding, smoking, drugs and alcohol.

| Eligibility | Women who are pregnant (or up to 6 weeks post delivery) and live in the Melton or Moorabool shires.
|
| Access hours | Monday – Friday 8am-4.30pm. All referrals to be made through Referral & Appointment Services on 97477609. |
| Referral process | Self referral or referral from other professionals in the community accepted.
