Service Description |
The Dietetic Service provides a full time community service for adults and children. All requests and referrals for information and consultation should be directed to the Intake worker at Melton Health who will take the relevant information, make an appointment or refer direct to the Dietician according to priority of request. Community Education, Health Promotion and Health screenings are offered through community groups, schools and the media. Focus is given to Government Health Promotion weeks and expressed community need. All age groups are catered for and key areas of work include clients with diabetes and heart disease. A holistic diabetes education service is provided, including education about diabetes, possible complications, glucose monitoring and lifestyle issues. Individual sessions and group work are conducted, often in cooperation with other health professionals such as the Podiatrist and Community Health Nurse; Groups include Healthy Cooking, Diabetes Education and Walking Groups. The Dietician also provides a consultancy service to the hospital and residential care units at the Bacchus Marsh and Melton Regional Hospital as well as staff from other agencies as requested. Community education is offered through schools, community groups and radio programs. A child and adolescent weight management clinic called ‘Ready Steady Go’ is available at Melton Health (see Paediatric Program) |

Referral process |
Community referrals: Self-referral or from General Practitioner, Medical Specialist or other Health Professional and carer. Hospital and Residential Care Services: request by Unit Manager by telephone or referral form sent direct to Intake. Requests should be directed to the Melton Community Health Centre for prompt attention. Initial contact (IC) will occur through the Intake worker for collation of information, appointments or direct referral to the Dietician as per designated priority of access. $15.00 per visit, $10.00 for HACC clients and Health Care Card holders Exemptions can be made following discussion with the Dietician. Workcover and Veteran Affairs’ clients – fees as directed. Melton Health – Bulk billed |
