Service Description

Services include:

• Comprehensive education for the newly diagnosed, including group and individual education

• Continuing education to all clients, tailored to their specific needs

• Education for women with gestational diabetes

• Instruction in use of blood glucose meter and insulin pen

• Assistance with ambulatory insulin stabilization

• Sale or loan of blood glucose meters

• Provision of insulin pens

• Provision of diabetes literature and videos

• Podiatry, Diabetes Nurse Educator, Dietitian

• Diabetes Support Group

Eligibility

People with diabetes or impaired glucose condition or family members of people with diabetes.

Cost to client

$15.00 per visit, $10.00 for HACC clients and Health Care Card holders. Exemptions can be made following discussion with the Dietitian or the Diabetic Nurse Educator.
