Service Description

The Chronic Disease Management program is available to those in the community who require care for lung and cardiac conditions and management of the co-morbidities associated with these long term medical conditions. This program may utilise the specialist services of the Respiratory Physician and Cardiologist in providing a condition specific rehabilitation service for these patients. The Cardiac and Pulmonary Rehabilitation clinic run under “best practice” guidelines set down by the National Heart Foundation and provides sessions for patients with pulmonary disease and for Phase 2 and Phase 3 cardiac patients. The Specialist service consultations fall under the General Medicine program.

Monday to Friday 9.00am to 4.30pm

Referral process

Referrals accepted by General Practitioners or self referral to the Chronic Disease Nurse.
