Service Description |
The Residential Aged Care Unit provides 24 hour quality nursing care in a home-like environment for thirty high care residents. Care is provided by fully qualified registered nurses, together with allied health services and diversional therapists. Home cooked meals with personal diet preferences noted are part of the holistic care. As part of the home like environment, Grant Lodge has its own pets. Eligibility for resident care is determined following assessment by an Aged Care Assessment Team. Any resident living in Bacchus Marsh, Melton or surrounding districts, or any resident who has a family member living in these areas, is eligible for placement. Once the assessment has been finalised, the Aged Care Assessment Team forwards details to the Manager. Prospective residents can, by appointment, view Grant Lodge. After completing the initial paperwork, the prospective resident’s name is put on the waiting list. Monthly updates of the waiting list are undertaken and these details are then forwarded to the Grampians Aged Care Assessment Service at Ballarat and North Western Aged Care Assessment Services. A Medical Practitioner of their choice can care for residents and their families. All residents and relatives are encouraged to participate in events occurring in the residential facilities and to become a part of the Grant Lodge life. |
