Djerriwarrh Health Services complies with the Information Privacy Act 2000 (Vic), the Health Records Act 2001 (Vic) and the Privacy Amendment (Private Sector) Act 2000 (Commonwealth).

Djerriwarrh Health Services is required by legislation to pass on some of the information it collects about people who receive services funded under our Health Service Agreement.  Information is sent to the Victorian Department of Human Services, and to the Commonthwealth Department of Health and Aged Care.  This is used for planning and statistical purposes.

No information which could identify an individual is forwarded to either State or Commonwealth departments.

What you can expect from us:

• As a user of our organisation, you can expect to be informed about the information we need to collect from you and why.
• We will take all reasonable steps to ensure that your information is accurate and up to date and is kept securely and confidentially.
• We will seek your consent before we provide your information to another organisation.
• We will only provide your information to another organisation where it will assist the provision of services to you.
• If your information needs to be transferred to another organisation this will be done securely and confidentially.
• In accordance with the Freedom of Information Act 1988, you have the right to access and correct your information held by us.
