| Service Description | Within a Harm Minimaization frameworks, provide assessment, support and case management to young people in their own wnvironment where appropriate support may include: -General information on substances -Develop harm reduction strategies -Develop Relapse prevention plan -Facilitated referral to withdrawal, rehabilitation and other specialists services -Court diversion program |
