Service Description

Carers of People aged over 65, frail aged people, people with dementia and people with disabilities, who reside in the Western Metropolitan Region of Melbourne.

Eligibility

Any person between the ages of 18-65 years (with some flexibility) who has an intellectual physical or sensory disability or acquired brain injury and who lives / works or can show significant links to the Shires of Melton or Moorabool.

Access hours

Arrival Time: Monday @ 2-4 pm – Departure Time: Thursday by 6.30 pm

Referral process

Priority of Access will be according to the assessed need of the carer and the care recipient
