| Service Description | Carers of People aged over 65, frail aged people, people with dementia and people with disabilities, who reside in the Western Metropolitan Region of Melbourne. |
| Eligibility | Any person between the ages of 18-65 years (with some flexibility) who has an intellectual physical or sensory disability or acquired brain injury and who lives / works or can show significant links to the Shires of Melton or Moorabool. |
| Access hours | Arrival Time: Monday @ 2-4 pm – Departure Time: Thursday by 6.30 pm |
| Referral process | Priority of Access will be according to the assessed need of the carer and the care recipient |
