Service Description

Provides respite for carers. Links with a range of support services to enable the caring relationship to continue e.g. home care, supported housing, assistance with daily living skills, home nursing, purchase of necessary equipment which cannot be funded from other sources.

Eligibility

Aged care, frail and chronic illness, Disability, mental health.

Access hours

9.00am – 5.00pm Monday/Friday.

Referral process

Community organisations, self-referral.
