Service Description

Multi-disciplinary team of professionals, which assists elderly people and their carers. The team consists of geriatricians, nurses, social workers, physiotherapists and occupational therapists. The team provides assessment, support and referral for health care, accommodation options, disability aids, respite care and other related issues.

Eligibility

Aged Care clients.

Access hours

9.00am –5.00am Mon-Fri.

Referral process

Referred by Medical Officer, Nursing staff, Allied Health or family. At Djerriwarrh Health Services referral to Western ACAS is for home visits only.
