Service Description

Provides suitable support services for people who are frail, aged and younger people with disabilities.

Services include specific home care, food services, transport, and activities for older people. Planned activity group (PAG), social support services, home care service, and respite service, and home maintenance are also provided.

Eligibility

Frail, aged and older people, people with an intellectual, physical or sensory disability, acquired brain injury.

Access hours

8.00am-6.00pm Mon/Fri.

Referral process

Self, relatives, health professionals and other agencies.

Cost to client

Cost varies depending on program required and income tested
