Service Description |
Post Acute Care describes the management and provision of short term planned care for patients and/or carers with more complex needs during a period of recuperation from an episode of acute illness. Short-term care (Health and Community Services) can be provided up to a maximum of 4 weeks. |

Eligibility |
Any person seen in either Primary Injury Service, Pre Admission Clinic or admitted to a public hospital. |
|
Access hours |
Daily 9.00am – 1200 noon. |

Referral process |
Referred by Medical Officer, Nursing staff or Allied Health. |
