Service Description

After discharge, a home visit is provided by a midwife for all maternity clients

Eligibility

All booked maternity families and referrals from other hospitals of any Moorabool and Melton families

Access hours

24 hours via the Maternity Unit

Referral process

Automatic for all in patients, via fax/telephone from other areas
