Service Description | Multi-disciplinary team of professionals, which assists elderly people and their carers. The team consists of geriatricians, nurses, social workers, physiotherapists and occupational therapists. The team provides assessment, support and referral for health care, accommodation options, disability aids, respite care and other related issues. |
Eligibility | Aged care clients. |
Access hours | Monday – Friday 9.00am – 5.00pm. |
Referral process | Referred by Medical Officer, Nursing staff or Allied Health. At Djerriwarrh Health services it is the responsibility of the Unit Manager or Discharge Planning Coordinator to refer. |
